beaux House Show Release Form

This agreement is made between Phzle Records and the undersigned individual ("the Host") regarding a live house show at the Host’s residence.

By signing this document, the Host agrees to the following:

Permission to Host

The Host consents to the Artist and their crew performing an acoustic live set in their home.

Filming & Photography Consent

The Host grants permission for the Artist and their videographer to film, photograph, and record the performance and attendees.
The Host understands that this content may be used for promotional purposes on social media and other platforms.

Age Requirements

The Host confirms that all individuals present in the home during the filming will be over the age of 18. If any individual under the age of 18 will be present, the Host must notify Phzle in advance so that the appropriate parental or guardian consent form can be provided.

Crew

The Host acknowledges that the Artist will be accompanied by a crew of up to three people: a band member, a videographer, and the Artist’s manager.

Performance Details

The performance will consist of two acoustic guitars only, with no amplification to minimise noise disturbance.
